Before or upon your first visit, please complete our new patient forms and gather any relevant dental records or X-rays. If you have dental insurance, bring or email your insurance card and information. Arrive a few minutes early to ensure a smooth check-in process.

We adhere to the highest standards of sterilization and infection control, following strict guidelines set by the CDC and ADA. All instruments are thoroughly sterilized, and our team uses personal protective equipment to ensure a safe, hygienic environment for our patients.

To maintain optimal oral health between visits, brush your teeth twice daily with a fluoride toothpaste, floss daily, and use an alcohol free mouthwash. Maintain a balanced diet, limit sugary and acidic foods and drinks, and attend regular check-ups and cleanings every 3 or 6 months at rootworks dentistry.
